Mrs. Mary Thela Formyduval Hooks, age 78 of Monroe and formerly of Old Dock died Saturday, August 21, 2010 at Presbyterian Hospital in Matthews. Final rites will be held 11:00 a.m. Tuesday, August 24, 2010 at Worthington Funeral Home in Chadbourn with Reverend Douglas Rumley officiating. Interment will follow in the Formyduval Family Cemetery, Whiteville. She was the widow of Edward Carol Hooks, Sr. and the daughter of the late Captain Willie & Ora Dorinda Evans Formyduval. She was also preceded in death by six siblings, Thurl Formyduval, Mort Formyduval, Faye Formyduval, Foy Formyduval, Ulysses Formyduval and Colene F. Mason. She is survived by two sons, Gary Hooks of Nakina and Edward Carol Hooks, Jr. of Monroe; two daughters, Wendy H. King of Nakina and Michelle Helms of Monroe; four brothers, Guy Formyduval, Lyle Formyduval, Wynn Formyduval, and Glean Formyduval all of Nakina; 6 grandchildren and 1 great-grandchild. The family will receive friends 7-9 p.m. Monday at Worthington Funeral Home in Chadbourn. No service information has been added.
