John "Jack" Gerome Formy-Duval

John “Jack” Gerome Formy-Duval, age 84 of Chadbourn, slipped into eternity on Tuesday morning, November 9, 2021, at Duke Raleigh Hospital.

Jack was born on September 26, 1937, in Columbus County, to the late James Cecil Formy-Duval and Carrie Emma Gore Formy-Duval. He was also preceded in death by his siblings, James “JC” Cecil Formy-Duval and Monte Formy-Duval.

Graveside services will be held at 11:00 a.m. on Saturday, November 13th, 2021, at Piney Forest Baptist Church Cemetery, 4509 Andrew Jackson Hwy SW, Chadbourn, NC 28431.  

Jack was a graduate of Whiteville High school where he was president of his class.  He earned a forestry degree from North Carolina State University and thus began his successful 37 year career with International Paperboard. Jack was also a veteran who served in the United States Army National Guard as a Lt. Colonel for 29 years.

The most important things in life were his faith, family, and friends.  He was a devoted husband, father, and grandfather.   He will truly be missed. 

Survivors include his wife of 60 years, Priscilla Faye Carter Formy-Duval of the home; daughter, Kelly F. Jones (Russell) of Wilmington; brother, Gene Formy-Duval of Southport; grandson, Sawyer Duval Jones. 

In lieu of flowers, the family has requested donations be made to Piney Forest Baptist Church in Chadbourn or the charity of your choice.  God bless you and your family,
